A dermatologist is commonly referred to as an urticaria

example, wheals of physical urticaria usually last less than one hour,
whereas those of ordinary urticaria typically last from two to
twenty-four hours. Urticarial vasculitis should be determined by
performing a skin biopsy if wheals last longer.

Most of the time, urticaria remains idiopathic after physical,
infectious, allergic, and drug-related causes have been excluded.
Nearly 30% of patients with the ordinary presentation of chronic
urticaria appear to have an autoimmune trigger.
- It
is common practice to augment the dose of second-generation H1
antihistamines above the manufacturer's recommendation for
patients when the possible benefits are considered to offset any risks.
- Diverse
combinations of non-numbing H1 antihistamines with a few other agents,
such as H2 sedating antihistamines at night, antihistamines, or the
addition of anti-leukotrienes, can be helpful for resistant cases.
- Some
immuno-modulating therapies for chronic autoimmune urticaria should be
restricted to patients with disabling diseases who have not responded
to optimal conventional treatments.
- The
advice of a qualified doctor on general measures and information can be
helpful for most patients with urticaria, especially if an avoidable
physical or dietary trigger can be identified. More than 40% of
hospitalized patients with urticaria show a favorable response to
antihistamines.
